This study tracked over 17,000 adults, averaging 57 years old, for an average of nearly eight years. Each person wore a wrist device for a week to capture real-world activity levels, and completed a cycling test to estimate cardiorespiratory fitness — essentially, how efficiently the heart, lungs, and muscles work together during exertion.

People who met the widely cited benchmark of 150 minutes of moderate-to-vigorous activity per week — things like brisk walking, running, or cycling — saw their cardiovascular risk drop by about 8% to 9%. That's a real benefit, and it held up consistently across fitness levels. But it's also a modest one relative to what showed up further along the activity curve.

Where the bigger payoff actually starts

The researchers, from Macao Polytechnic University, defined "substantial protection" as more than a 30% reduction in cardiovascular risk. That level of benefit was associated with 560 to 610 minutes (9.3 to 10.1 hours) of moderate-to-vigorous activity per week — a meaningfully higher bar than the standard guideline, and one only about 12% of participants actually reached.

That's a useful reframe: the 150-minute guideline looks less like a target to stop at and more like a floor. The researchers themselves described it as a reliable minimum for basic protection, distinct from the higher volume linked to optimal benefit.

Fitness level changes the math, a little

The study also looked at whether a person's baseline fitness changed how much exercise they needed. People with lower cardiorespiratory fitness needed somewhat more activity than highly fit participants to reach a comparable benefit — roughly 30 to 50 extra minutes per week for the same size of risk reduction. The researchers called this a "steeper challenge" for people starting from a more deconditioned baseline, which is a fair way to think about it: the same finish line, just a slightly longer run-up for some people.

A longevity lens: guidelines as a floor, not a ceiling

This is a good example of how public-health guidelines and personal optimization goals can diverge without contradicting each other. A guideline has to be achievable for a whole population, so it tends to describe a reasonable minimum. But for someone actively investing in their long-term healthspan, this data suggests there's real additional upside in pushing well past that minimum — provided it's sustainable and doesn't crowd out recovery, sleep, or other pillars of a balanced routine.

The researchers also floated the idea of personalized activity targets based on individual fitness level, rather than one number for everyone. That's consistent with where a lot of longevity thinking is heading: less "hit this universal number" and more "understand your own baseline and build from there."
